PAYMENT TERMS
^Membership fees are due monthly unless otherwise specified.^Auto-pay authorization may be required^Membership fees are non-transferable^Returned or failed payments may result in additional fees or suspension of membership access
CANCELLATION POLICY
^Membership cancellation requires written notice at lease 14 days before next billing cycle. ^No partial refunds for unused time^Memberships may be paused only with management approval^Failure to follow facility rules may result in termination without refund
FACILITY RULES
Members agree to:
^Follow all facility safety rules^Respect staff, coaches, players, and guests^Use equipment properly^Maintain good sportsmanship and conduct^Supervise minors when not participating in instruction
Violation of rules may result in suspension or termination of membership privileges.

ASSUMPTION OF RISK
I understand that participation in baseball and softball activities involves inherent risks including injury, permanent disability, or death.

RELEASE OF LIABILITY
I release and hold harmless Country Field House, LLP, its owners, employees, instructors, volunteers, and affiliates from any liability arising from participation, facility use, or related activities.
PHOTO & VIDEO RELEASE
I grant permission for photographs / videos taken during activities to be used for promotional and marketing purposes unless written notice is provided.
